Pharmacological action:
Orlistat is a potent, specific, and reversible long-acting lipase inhibitor. It exerts its therapeutic effect in the stomach and small intestine by forming a covalent bond with the serine residue of the active site of gastric and pancreatic lipases. As a result, the inactivated enzyme cannot hydrolyze dietary fat in the form of triglycerides into absorbable free fatty acids and monoglycerides. Since undigested triglycerides are not absorbed, the resulting calorie deficit has a positive effect on weight control.
Indications:
Orlistat is used for weight management, including weight loss, weight maintenance, and prevention of weight regain in adults with an initial body mass index of 30 or greater.
Orlistat should be used in conjunction with a low-fat, calorie-controlled diet.
Dosage: The recommended dose for adults is 120mg three times a day, one capsule per dose, taken within one hour after a meal (or up to one hour after a meal).
Patients should follow a nutritionally balanced, low-calorie diet, with approximately 30% of calories derived from fat. The diet should emphasize fruits and vegetables. Daily intake of fat, carbohydrates, and protein should be distributed across three meals. No dose adjustment is needed for elderly patients.
Contraindications:
Orlistat capsules are contraindicated in patients with chronic malabsorption syndrome, cholestasis, or known hypersensitivity to orlistat or any of the ingredients.
Warnings and Precautions:
In studies lasting up to 4 years, most patients had normal levels of vitamins A, D, E, and K, as well as beta-carotene. To ensure adequate nutrition, supplementation with a multivitamin should be considered.
The likelihood of gastrointestinal events increases when orlistat is taken with a high-fat meal (e.g., a 2000-calorie diet with >30% of calories from fat, equivalent to >76g of fat). Daily fat intake should be distributed across three meals. If orlistat is taken with any high-fat meal, the possibility of gastrointestinal reactions may increase. If a meal is skipped, the corresponding dose of orlistat should also be omitted.
Weight loss induced by orlistat is achieved through improved metabolic control, so in patients with type 2 diabetes, the dose of hypoglycemic medications (e.g., sulfonylureas) may need to be reduced.
